Transmission
- Contacto con orina, heces o saliva de ratas multimamarias infectadas (Mastomys natalensis)

Symptoms
Mild to moderate (majority of cases)
- Gradual fever onset (≥38 °C / 100.4 °F)
- General malaise and fatigue
- Headache, often retro-orbital
- Sore throat (pharyngitis, sometimes with white exudate)
- Muscle and joint pain (myalgia, arthralgia)
- Nausea, vomiting, and diarrhea
- Chest and abdominal pain
Severe disease (~20% of symptomatic cases)
- Facial and neck swelling (edema)
- Haemorrhagic manifestations — bleeding from gums, nose, or gastrointestinal tract
- Respiratory distress and chest tightness
- Altered consciousness, encephalopathy, or seizures
- Shock — hypotension, cold clammy skin
- Sensorineural hearing loss: affects 25-30% of survivors, may be permanent
Key clinical differentiators
- Deafness during or after recovery — unique among viral haemorrhagic fevers
- Proteinuria (protein in urine) — early marker of renal involvement
- Pharyngitis with exudate is a distinctive early sign
- 80% of infections are asymptomatic or very mild — less dramatic clinically than Ebola

Treatment
La ribavirina es eficaz si se administra de forma temprana — idealmente en los primeros 6 días desde el inicio de la fiebre — junto con cuidados de soporte (fluidos IV, corrección de electrolitos, tratamiento de infecciones secundarias). La pérdida auditiva neurosensorial afecta al 25-30% de los pacientes y puede ser permanente.
Vaccine status
No hay vacuna contra la fiebre de Lassa aprobada por la OMS a 2026. Varios candidatos (p. ej. INO-4500, MV-LASV, vacunas vectorizadas con rVSV y sarampión) están en desarrollo clínico, varios con financiación de CEPI.
